As a courtesy to other patients who need to be seen, if you need to cancel your appointment please notify us at least 24 hours in advance. If we are not contacted 24 hours before your appointment, then the following fees will apply:
- $25 for Existing Patient (NO SHOW & LATE CANCELLATION)
- $75 for New Patients (NO SHOW & LATE CANCELLATION)
School forms can be emailed to us at [email protected]. Please allow 5-7 business days for all forms to be completed. Once they have been completed our office may fax or mail your forms back to you. School forms are $5.00 per page. FMLA forms are $15.00. Payment must be collected before your forms are faxed or emailed back to you. Payments are accepted over the phone or in the office if you choose to pick your forms. Please review our new School Form Policy which came into effect August 1, 2021.
