Patient Forms

Please download and email your completed New Patient Registration Packet, along with any medical records chest or sinus x-rays before your appointment. Our email is [email protected]. As a courtesy to other patients who need to be seen, if you need to cancel your appointment please notify us at least 24 hours in advance. If we are not contacted 24 hours before your appointment, then the following fees will apply:

  • $25 for Existing Patient (NO SHOW & LATE CANCELLATION)
  • $75 for New Patients (NO SHOW & LATE CANCELLATION)

SCHOOL FORMS

School forms can be emailed to us at [email protected] Please allow 72 hours for all forms to be completed. Once they have been completed our office may fax or mail your forms back to you. You may also come to our office to pick up your forms. Please review our new School Form Policy which came into effect October 1st, 2019.
