If you would like, you can download the PDF, complete the form, and email the completed form to us along with any medical records chest or sinus x-rays before your appointment.
As a courtesy to other patients who need to be seen, if you need to cancel your appointment please notify us at least 24 hours in advance. If we are not contacted 24 hours before your appointment, then the following fees will apply:
- $25 for Existing Patient (NO SHOW & LATE CANCELLATION)
- $75 for New Patients (NO SHOW & LATE CANCELLATION)
ALL FORMS NEED TO BE PRE-PAID BEFORE THEY ARE COMPLETED! FORMS TAKE 7 BUSINESS DAYS TO COMPLETE!
There will be a $10 fee for each form completed! FMLA paperwork is $25
For Allergy and Asthma to complete the form(s) the following is required: Your child must have had an appointment within the last 12 months. Completion of parent/child portion of each form with name, date of birth, medication(s) and current weight. Please indicate how you would like to receive the forms back once completed. You can pick them up from the office, have them faxed or mailed. If you would like them mailed, please provide a self addressed stamped envelope. Forms must be emailed to fro[email protected] or brought in personally to the office.
